Description:
Position Summary:
A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served who also serves as a support to Community Health Workers.
This trusting relationship enables the Lead CHW to serve as a liaison/link/intermediary between the Community Health Workers and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
A Lead CHW acts as a mentor to Community Health Workers on their team.
What you will do:
Enrolls clients in available federal, state and local programs to increase access to health care, healthy food and social service programs.
Conducts home visits as outlined in the Health Project’s health navigation program.
Conducts outreach efforts to community and neighborhood organizations to promote the availability of Health Project Programs.
Conducts client eligibility interviews and makes appropriate referrals to other services and programs.
Manages client records and inputs necessary information into designated information systems in a timely manner.
What you need:
Fluent in Spanish and English preferred
Community Health Worker Certified, preferred.
Two years of experience in outreach and enrollment work is preferred.
Familiarity with local health and human resources
GED/high school diploma required
