Position Summary:
Is responsible for operational, financial, and clinical operations of Social Services Department including Social Work Case Managers, Behavioral Health Practitioners, and Spiritual Care Staff.
This position leads efforts in identifying improvement opportunities and innovations, designed to positively impact participant care while ensuring that policies and procedures are implemented consistently across sites and that utilization is consistent with the PACE model of care.
Position Functions and Duties:
Provides leadership to social services department, focusing on the core values of the PACE Model of Care and ensuring all members of the department are providing the highest level of quality care to our participants
Ensures alignment of processes that support care delivery and financial responsibility across both sites.
Responsible for monitoring, and analyzing, utilization of resources and developing plans to remain fiscally responsible
In collaboration with other directors, ensures that Social Services policies and procedures are in alignment with regulations and organizational priorities.
Develops and plans for a department structure that supports growth, participants needs, and provides staff with the professional development resources to work at the top of their licensure.
Provides clinical guidance to the department to ensure social work and behavioral health interventions are effective and in line with established best practices.
Appropriately collaborates and coordinates work with other Directors to ensure departments are working together and towards the same goals.
Provides performance based feedback, accountability, and professional development opportunities for staff
Assists with relationship building and problem solving with contracted providers.
Uses data to identify priorities and process improvement initiatives.
Demonstrates an ability to prioritize tasks and initiatives
Works with direct reports to ensure the care being delivered aligns with the PACE model of Care.
Position Qualifications:
Minimum of 7 years of progressive leadership experience
MSW or Advanced Degree in Social Service related field
Experience in Project Management
Adaptable and flexible to changing priorities and a dynamic work environment
Strong desire to collaborate with other leaders to achieve the best outcome
